Registration & Membership
- I agree to pay an annual membership fee and subs of £190 to cover the season from 23 August 2024 to 31 May 2025. The upfront membership fee is £50 followed by 5 monthly payments of £28, totalling £190.
- Any player registering after the start of the season will still pay the registration fee in full.
- Unfortunately we can’t refund any money as a result of games being cancelled for bad weather.
- All registration forms will be accepted unless otherwise informed.
- A member whose annual membership fee or further subscription has not been paid in full by 31st December shall be deemed to have resigned, unless otherwise agreed with the committee.
Data & Privacy
- The information and data which is provided by you during registration will be held within Spond, a club management tool.
- Weetwood Blaze FC may communicate with you from time to time by email, telephone or mail about relevant news, information, or events run by the club.
- Data will be managed in line with the club’s Data Protection Policy, which is available on the club website.
- The club does not take out player insurance, we do have public liability insurance which we acquire through the FA.
Responsibilities
- On signing this form both players and parents agree to abide by the rules of the Harrogate and Craven FA, your appropriate league, Weetwood Blaze constitution AT ALL TIMES (rules and constitutions are available on the relevant clubs, leagues and WRCFA websites or from the club secretary).
- If my child suffers from a medical condition that requires medication, I agree to supply the team manager with this medication and agree to my child being given this medication if I am not available.
- I give my consent for my child/myself to receive medical attention if necessary either first aid by a qualified first aider, or hospital treatment.
- If I/we the child’s parents are unavailable, I agree to the manager, assistant manager or coach acting in ‘loco parentis’.
- It is the responsibility of Parents/Guardians to arrange transport of their children to training sessions and football games.
- Team members should attend training sessions and football games prepared for outdoor activity.
- All team members are asked to wear shin guards at all training sessions and football games.
- All team members are asked to bring water bottles to stay hydrated and waterproofs for adverse weather.
- I understand that Weetwood Blaze FC and/or its appointed representatives eg. officers, managers, coaches, are under no liability and cannot be held responsible for any loss, damage or injury suffered by my child, however caused, whilst taking part in any club matches, activities or events etc including travelling to/from such occasions.
- We reserve the right to remove from our teams without refund, any participant who is found bullying, misbehaving, or displaying disruptive behaviour.
- Weetwood Blaze FC are not responsible for team members prior to or after training sessions or football games
- Football kit is provided by sponsors. The football kit, consisting of shirt, shorts and socks and any other equipment/clothing which may be supplied from time to time
Termination
- This agreement can be terminated at any time
- Parents agree to all conditions by signing on their child to Weetwood Blaze FC for coming Season 2024-25
